Judging from the "you'd better not get this wrong" look on my wife's face, I'd suggest either knowing said date, or, filling out the form when the spouse is not present.😉

I made it easy on myself. Her birthday and our anniversary month start with the same letter and share identical dates. I just have to remember which card to give her.
