I, too, haven't seen anything in the shape of a reward yet. I participated in two different tests and have still not heard from anyone in any way, shape or form.
However, I must admit playing all the expensive prototypes is quite entertaining. Plus, it gives me a risk free environment to test all my dream builds before I have to cough up all the slow earned AU for a specific build.

Rewards seem to be hit or miss. I have submitted feedback 4 times, and got rewards only 2 times. I used the same ID all 4 times, and made certain it was correct.

Update... Now 3 out of 5 feedbacks, I got rewards.
However, this time I posted a video, and never got any extra rewards that they mention.

Got mine, 24 hours premium and either 100 or 200 gold, forgot to check before it went up but I swear it's more than I had.

Update: got my second reward. 100 gold plus 24h of premium. No announcement, notification, or popup bubble whatsoever.

I didn't get squat.. did the feedback and a video.. Two videos actually. I won't bother with any of that jump through hoops again.

I don't play test server anymore, the update is coming too fast, and size for update is too big it's like more than 300 MB.

Hi. I'm new to test servers. To get rewards in the real game, do I have to post a video, or just give feedback(take the survey)? And does the video have to be on YouTube?

The new PIX standard to getting rewards is to submit feedback on the test server (do this before Monday). Submit a link to video of the test server play, I assume Youtube would be the best avenue.
